Applications for professional membership are accepted directly by the Membership Department at OTHM's London office.

If you wish to study for an OTHM qualification, you must first become a Student Member. Applications for Student Membership will only be accepted through a recognised OTHM course delivery centre.

Saving these Forms
Each form is in Rich Text Format (RTF). RTF is used because this file format can be read by most word processors.
PC Users: right-click on the download link and a menu appears
or
Mac Users: click and hold on the download link until a menu appears
From the menu select 'Save Target As...' or 'Save Link As'
A box will appear inviting you to select a location on your computer where you can save the file. Select the location you want and click 'OK'
Another box will appear with information about the download progress.
Editing these Forms
When the file has been downloaded, open the form using your word processor
Please try to complete as much of the form as possible using your word processor. If your application form is incomplete we will have to contact you for the information, which slows down the processing.

Make sure that you have signed the form where indicated.
If you are applying to take a course, please send your completed Student Application form to the training centre or college of your choice.
